Instructions: As required in Chapter 29 Rules, any district leader evaluation system that does not align to six of the seven state-defined leader evaluation professional standards must be submitted and approved by the State Board of Education (SBE). The Certified Personnel Evaluation System Committee crosswalked a number of superintendent and principal evaluation systems you may use as a reference. Districts may also submit additional evaluations for other district and school leaders. If you are submitting more than one evaluation system for approval, this form will need to be submitted for each evaluation. All evaluations submitted to the SBE will need to complete a crosswalk showing alignment between Standard 1 of the State-Defined Leader Evaluation Model and the district’s leader evaluation.
